What the forms collect

Contact forms collect your name, contact details and enquiry. Referral forms also collect the participant’s name, location, requested support, goals and any optional information you supply. Careers forms collect your work interests and experience. Feedback forms collect your comments and any contact details you choose to include.

Why this information is collected

The information is saved for Passion Services Care to review and manage your enquiry, referral, expression of interest or feedback. Information is not displayed publicly on the website. Website forms do not send a copy to an email inbox automatically.

Sharing information about someone else

Only provide another person’s information if you have their consent or lawful authority. Share the minimum information needed for an initial discussion. Do not include identity documents, financial details or full clinical records.

Anonymous feedback

Selecting anonymous feedback removes the contact fields and saves the feedback without them. Avoid including details in your message that identify you if you want to remain anonymous. The website hosting service may process ordinary technical request information to operate the site.

Access, corrections and questions

Use Contact Us to request access to, correction of or deletion of information you submitted, or to ask how it is handled. Include your submission reference where available. Identity or authority may need to be confirmed before a request is actioned.

Each form asks you to consent to the use of your information for that request. You can raise privacy concerns through Feedback & Complaints.
